Has anyone been able to get the CASIsapi filter to work?

The CASIsapi filter fails to load for me. I've been stuck on this for a while now and would very much appreciate some help. Nothing is logged to the system, even if I set 'loggingMask' to 999 in the casisapi.config file. I've also tried loading it as a 'web service extension' which appears to do nothing. I've gotta say that I'm not familiar with IIS so I could be getting something basic wrong. I've tried my best to follow the attached instructions, and then tried just about everything else I can see that might have an effect, but with no success. Please can somebody help me... I'm getting desperate... oh, yes... and it's not fun any more!
