CASAuthNHeader purpose in mod-auth-cas

We have run into a few (aggravating) applications that do not use
REMOTE_USER, but rather require a special, application-specific header
instead. CASAuthNHeader allows for this.

Of course, I do believe mod_rewrite can handle this too -- but I don't
want to get into Apache-module-loading-order-dependencies.
