I've noticed it inconsistently (i.e. rarely) but I don't have an appropriate
test environment.  If anyone can narrow this bug down even further that
would great!

SAML identifies the person by whatever your Principal's name is that is
returned from the CredentialsToPrincipalResolver so I'm not sure what you
mean.
